How does Bruce willis not know that he's dead in the sixth sense?

Just a lot of things don't add up. How does he make an appointment with the kid if he's dead? And don't you think he wouldve figured it out after a half year of not talking to his wife? How would he have been "released" from the hospital? Anyone have an answer?
